About Castleman Disease (HHV-8 Associated)

Castleman disease, HHV-8 associated, is a rare disorder involving overgrowth of lymph node cells, linked to the Human Herpesvirus 8. It often affects individuals with weakened immune systems. Managing this condition requires specific treatments, which can be costly. TheDrugClub helps make these vital prescriptions more affordable, providing peace of mind to patients and their families.

Castleman disease (CD) is a rare condition that causes an overgrowth of cells in the lymph nodes, similar to lymphoma. Among its different forms, HHV-8 associated Castleman disease (HHV-8 MCD) is a distinct and often more aggressive type, specifically linked to the Human Herpesvirus 8 (HHV-8). This form is particularly common in individuals with compromised immune systems, such as those living with HIV.

What is HHV-8 Associated Castleman Disease?

Unlike other forms of CD, HHV-8 MCD is driven by the HHV-8 virus, which stimulates specific immune cells (B cells) to multiply uncontrollably in the lymph nodes and other organs. Symptoms can be widespread and severe, including persistent fever, night sweats, unexplained weight loss, fatigue, and enlarged lymph nodes in areas like the neck, armpits, or groin. It can also affect internal organs, leading to issues with the liver, spleen, and bone marrow. Diagnosis typically involves a lymph node biopsy, blood tests to detect HHV-8, and imaging scans to assess disease extent.

Common Treatments and Medications

Treatment for HHV-8 MCD focuses on suppressing the virus, reducing inflammation, and managing symptoms. Because the condition is driven by HHV-8, therapies often target the virus or the immune pathways it activates. Common medications include:

  • Monoclonal Antibodies: Drugs like siltuximab (which targets IL-6, a cytokine overproduced in HHV-8 MCD) and rituximab (which targets B-cells, often infected by HHV-8) are frequently used. These can be highly effective but also very expensive.
  • Antiviral Medications: While their direct impact on HHV-8 MCD itself is debated, antivirals like ganciclovir or valganciclovir may be used, especially in patients with high viral loads or co-infection with HIV.
  • Chemotherapy: In more aggressive cases or when other treatments are ineffective, chemotherapy regimens might be employed.
  • HIV Treatment: For patients co-infected with HIV, optimizing their antiretroviral therapy is a critical part of managing HHV-8 MCD.
