About hCG Plateauing Post-Mole

A molar pregnancy is a rare complication that occurs when there's abnormal growth of cells in the uterus after conception. While often treated with a procedure to remove the abnormal tissue, close follow-up with blood tests to monitor HCG levels is essential. HCG is the hormone produced during pregnancy, and after a molar pregnancy, these levels should steadily decline until they are undetectable.

However, sometimes HCG levels don't fall as expected; instead, they might stabilize (plateau) or even start to rise again. This is known as an HCG plateau post-mole. It’s a critical indicator that some abnormal cells may still be present, a condition referred to as persistent gestational trophoblastic disease (GTD). While this can be concerning, it’s important to remember that GTD is highly treatable, especially when detected early.

Common Treatments and Medications

When an HCG plateau or rise is confirmed, treatment typically involves chemotherapy. The specific medications and regimen depend on various factors, including the type of GTD and whether it has spread. Common first-line chemotherapy drugs include Methotrexate or Actinomycin D, often given as single agents. For more resistant cases or those with higher risk factors, multi-drug regimens might be used.

These medications are generally very effective, leading to a complete resolution of the condition in most patients. Regular HCG monitoring continues throughout treatment and for a period afterward to ensure complete remission. Supportive care medications may also be prescribed to manage potential side effects of chemotherapy.
