About IgG4-Related Disease (Kidney)

IgG4-related kidney disease (IgG4-RKD) is an inflammatory condition that can significantly impact kidney function, often requiring ongoing medical management. Understanding this complex condition and its treatments is crucial for patients. TheDrugClub is dedicated to helping individuals and families afford the vital medications needed for IgG4-RKD, ensuring access to necessary care without financial strain.

What is IgG4-Related Kidney Disease?

IgG4-related kidney disease (IgG4-RKD) is a manifestation of IgG4-related disease (IgG4-RD), a systemic immune-mediated fibroinflammatory condition characterized by an abnormal accumulation of IgG4-positive plasma cells and fibrosis in various organs. When it affects the kidneys, it can lead to various forms of kidney injury, most commonly tubulointerstitial nephritis, but also membranous nephropathy or obstructive nephropathy due to involvement of the ureters. Patients might experience symptoms like fatigue, swelling, or abnormal lab results indicating impaired kidney function. Diagnosis often involves a kidney biopsy to identify the characteristic IgG4-positive plasma cell infiltration and fibrosis, alongside elevated serum IgG4 levels and imaging findings. Because IgG4-RKD can mimic other kidney diseases, accurate diagnosis is key for effective treatment.

Common Treatments and Medications

The primary goal of treating IgG4-RKD is to reduce inflammation, prevent further organ damage, and preserve kidney function. The cornerstone of treatment typically involves corticosteroids, such as prednisone. These medications work by suppressing the immune system and reducing inflammation. Patients often start with a high dose, which is then gradually tapered over several months.

For those who don't respond adequately to steroids, experience frequent relapses, or require steroid-sparing options due to side effects, other immunosuppressive agents may be used. These include medications like rituximab, azathioprine, or mycophenolate mofetil. Rituximab, a monoclonal antibody, targets B cells, which play a role in the immune response. Given the chronic nature of IgG4-RKD, long-term management
