About MHC Class II Deficiency (Bare Lymphocyte Syndrome)

MHC Class II Deficiency Bare Lymphocyte Syndrome (BLS) is a rare genetic immune disorder leaving individuals vulnerable to severe, recurrent infections. Managing this lifelong condition often involves costly treatments and medications, placing a significant burden on families. TheDrugClub offers a vital solution, helping members significantly reduce prescription expenses and access necessary care for BLS.

MHC Class II Deficiency Bare Lymphocyte Syndrome (BLS) is a rare, inherited primary immunodeficiency characterized by the body's inability to produce major histocompatibility complex (MHC) class II proteins. These proteins are crucial for T-cells, a type of white blood cell, to recognize foreign invaders like bacteria and viruses. Without functional MHC class II molecules, the immune system cannot properly activate and mount an effective defense, leading to a severe combined immunodeficiency (SCID)-like state.

Individuals with BLS are highly susceptible to severe, recurrent infections, particularly affecting the respiratory and gastrointestinal tracts. These infections can be life-threatening if not managed aggressively. The condition typically manifests in infancy with symptoms such as chronic diarrhea, failure to thrive, persistent viral and bacterial infections, and opportunistic infections. Diagnosis usually involves specialized genetic testing and flow cytometry to confirm the absence of MHC class II molecules on immune cells.

Treatment for BLS focuses on managing infections and, ideally, correcting the underlying immune defect. The most effective and potentially curative treatment is hematopoietic stem cell transplantation (HSCT), often referred to as a bone marrow transplant. This procedure replaces the faulty immune system with healthy stem cells from a donor. However, HSCT is complex, carries significant risks, and is extremely expensive. For patients awaiting or unable to undergo HSCT, supportive treatments include regular intravenous immunoglobulin (IVIG) or subcutaneous immunoglobulin (SCIG) therapy to provide passive immunity, as well as prophylactic antibiotics, antifungals, and antivirals to prevent infections. These ongoing medications are vital but can accumulate to substantial costs over time.
