Lost the Paperwork That Came With Your Prescription? How to Ask for a Copy
· 1 min read
Misplaced the information sheet or receipt from a fill? Here's how to ask your pharmacy for another copy, and when each one matters.
Short answer: If you lost the information sheet or the receipt from a fill, ask the pharmacy for another copy. Give your name, date of birth, the medicine, and roughly when it was filled. For medication questions, you can also ask to speak with the pharmacist.
The information sheet
The printed sheet that comes with a prescription explains what the medicine is, how to take it, and what to watch for. If you lost it:
- Ask the pharmacy: "Could you print another copy of the information sheet for [medicine]?"
- If you have questions, ask the pharmacist directly. See getting time with the pharmacist.
The receipt
Receipts matter for your records. To get a copy:
- For several fills, ask for a prescription history printout that shows amounts paid.
Our guide to reading a pharmacy receipt explains what's on it.
Have these ready
- Full name and date of birth
- Medicine name and strength
- Approximate fill date
- Rx number from the label, if you have it
Keep them together next time
A folder or an envelope for each person in your household keeps sheets and receipts in one place. A photo of each receipt is a good backup.
Paperwork checklist
- Information sheet reprint requested
- Receipt copy requested with the fill date
- History printout requested for multiple fills
- Pharmacist asked any medication questions
- Folder or photo backup set up
