About Cirrhosis with Spontaneous Bacterial Peritonitis
Cirrhosis, a late stage of liver scarring, significantly impairs liver function. One of its most dangerous complications is Spontaneous Bacterial Peritonitis (SBP), a serious infection of the fluid that accumulates in the abdomen, known as ascites. Unlike other abdominal infections, SBP typically occurs without any obvious source, such as a ruptured organ. It's a medical emergency, as bacteria from the gut can translocate into the ascitic fluid, leading to a rapid and severe infection that can be fatal if not treated immediately.
Patients with cirrhosis and SBP often experience symptoms like fever, abdominal pain, tenderness, and worsening liver function. Diagnosis involves drawing a sample of the ascitic fluid (paracentesis) for analysis to confirm the presence of infection. Given its severity, early diagnosis and treatment are crucial for survival.
Common Treatments and Medications for SBP
The primary treatment for SBP is antibiotics. Initial treatment typically involves broad-spectrum intravenous (IV) antibiotics, such as cefotaxime or ceftriaxone, to target a wide range of potential bacteria. Patients may also receive albumin infusions, which can help prevent kidney failure, a common and serious complication of SBP. Once the infection is under control, patients often require long-term oral antibiotics (e.g., norfloxacin, ciprofloxacin, or trimethoprim-sulfamethoxazole) as prophylaxis to prevent recurrence, as SBP has a high rate of coming back.
Managing cirrhosis and its complications, including SBP, often means a lifetime of medication. These prescriptions, from initial high-dose IV antibiotics to ongoing preventive oral medications and other drugs for liver support, can quickly become a significant financial strain.
