Hemophilia A is a rare, inherited bleeding disorder caused by a deficiency in clotting Factor VIII. For many, treatment involves regular infusions of Factor VIII to prevent or control bleeds. However, a significant complication can arise: the development of inhibitors.
An inhibitor is an antibody produced by the immune system that recognizes the infused Factor VIII as a foreign substance and neutralizes it. This makes standard Factor VIII replacement therapy ineffective, leading to more frequent, severe, and difficult-to-control bleeding episodes. These bleeds can occur in joints, muscles, and even vital organs, potentially causing long-term damage, chronic pain, and even life-threatening situations.
Treating Hemophilia A with Inhibitors
Managing hemophilia A with inhibitors requires a different approach. Treatment strategies typically fall into a few categories:
- Bypassing Agents: For acute bleeding episodes, medications like activated prothrombin complex concentrate (aPCC) or recombinant Factor VIIa (rFVIIa) are used. These agents bypass the need for Factor VIII and help promote clotting.
- Immune Tolerance Induction (ITI): This intensive therapy involves giving high doses of Factor VIII over an extended period to try and "retrain" the immune system to tolerate the factor. ITI can be successful in eradicating inhibitors for many patients, but it is a lengthy and costly process.
- Non-Factor Therapies: Newer medications, such as emicizumab, offer a preventative (prophylactic) approach. These therapies work differently than traditional factor replacement and are designed to prevent bleeds in people with inhibitors, improving quality of life.
