About Hyper-IgM Syndrome Type 1
Hyper IgM Syndrome Type 1 (HIGM1) is a primary immunodeficiency disorder, meaning it’s a condition present from birth that impairs the immune system’s ability to fight off infections. It's caused by a genetic mutation, most commonly on the X chromosome, which affects the CD40 ligand protein found on T cells. This protein is crucial for B cells to switch from producing IgM antibodies to producing other, more effective types like IgG, IgA, and IgE.
Because B cells can't perform this class switching, individuals with HIGM1 have abnormally high levels of IgM antibodies but very low or absent levels of IgG and IgA. This leaves them vulnerable to recurrent and often severe bacterial, viral, and fungal infections, particularly in the respiratory and gastrointestinal tracts. Common challenges include pneumonia, sinusitis, ear infections, and opportunistic infections like Pneumocystis jirovecii pneumonia (PJP). Diagnosis often occurs in infancy or early childhood due to these frequent and serious illnesses.
Treatments for HIGM1 primarily focus on preventing and managing infections. The cornerstone of therapy is Immunoglobulin Replacement Therapy (IRT), administered either intravenously (IVIG) or subcutaneously (SCIG). This provides the body with the missing antibodies (IgG) to help fight off pathogens. Patients may also require prophylactic antibiotics or antifungals to prevent specific infections, and in some cases, granulocyte colony-stimulating factor (G-CSF) for neutropenia. For a select group of patients, a hematopoietic stem cell transplant (HSCT) can offer a potential cure, though it carries significant risks. These treatments, especially IRT, are often lifelong and can be incredibly expensive, creating a substantial financial burden for families.
