About Indomethacin (Tocolytic)

Indomethacin, when used as a tocolytic, plays a crucial role in the short-term management of preterm labor, offering a window to improve fetal outcomes. Understanding this medication is key for expectant parents and healthcare providers. At TheDrugClub, we believe in making essential medications more accessible and affordable, helping our members save significantly on prescriptions like indomethacin.

Indomethacin is a nonsteroidal anti-inflammatory drug (NSAID) that finds an important, albeit off-label, use as a tocolytic. A tocolytic is a medication designed to suppress uterine contractions and delay preterm labor. This critical delay can provide valuable time for interventions like administering corticosteroids to the mother, which helps accelerate fetal lung development, or facilitating a safe transfer to a facility equipped for high-risk deliveries. It is typically used for short durations, usually no more than 48-72 hours, and generally between 24 and 32 weeks of gestation, due to potential risks to the fetus with prolonged use or use later in pregnancy.

How Indomethacin Works to Delay Labor

The mechanism behind indomethacin's tocolytic action lies in its ability to inhibit prostaglandin synthesis. Prostaglandins are hormone-like compounds that play a significant role in initiating and maintaining labor contractions. By blocking the enzymes cyclooxygenase-1 (COX-1) and cyclooxygenase-2 (COX-2), indomethacin reduces the production of these prostaglandins in the uterus. This reduction in prostaglandin levels decreases the intensity and frequency of uterine contractions, thereby helping to delay preterm labor.

Typical Costs of Indomethacin

The cost of indomethacin can vary based on the dosage, quantity prescribed, the specific pharmacy, and your geographic location. As a generic medication, indomethacin is generally more affordable than brand-name alternatives. Without insurance or a discount program, a typical prescription for a short course of indomethacin might range anywhere from $20 to $60, though these are estimates and prices can fluctuate. These costs can be a concern

Drug interactions for Indomethacin (Tocolytic) (FDA label)

See Table 2 for clinically significant drug interactions with indomethacin. Table 2 Clinically Significant Drug Interactions with Indomethacin Dr ugs That Interfere with Hemostasis C linica l Impact: Indomethacin and anticoagulants such as warfarin have a synergistic effect on bleeding. The concomitant use of indomethacin and anticoagulants have an increased risk of serious bleeding compared to the use of either drug alone. Serotonin release by platelets plays an important role in hemostasis. Case-control and cohort epidemiological studies showed that concomitant use of drugs that interfere with serotonin reuptake and an NSAID may potentiate the risk of bleeding more than an NSAID alone.
