About Primary Biliary Cholangitis

Primary Biliary Cholangitis (PBC) is a chronic, progressive autoimmune liver disease that slowly damages the bile ducts. While there's no cure, effective treatments can manage symptoms and slow its progression. TheDrugClub is dedicated to making these essential medications more affordable for our members, helping you focus on your health without the added financial stress.

Understanding Primary Biliary Cholangitis (PBC)

Primary Biliary Cholangitis (PBC) is a chronic liver condition where the small bile ducts within the liver become inflamed and are slowly destroyed. These bile ducts are crucial for transporting bile, a digestive fluid, from the liver to the small intestine. When they are damaged, bile builds up in the liver, leading to inflammation, scarring (fibrosis), and eventually, cirrhosis – severe scarring that impairs liver function. PBC is an autoimmune disease, meaning the body's immune system mistakenly attacks its own healthy cells.

Symptoms often develop gradually and can include debilitating fatigue, intensely itchy skin (pruritus), dry eyes and mouth. As the disease progresses, some individuals may experience jaundice (yellowing of the skin and eyes), fluid retention, and bone problems. Diagnosis typically involves blood tests, particularly for antimitochondrial antibodies (AMAs), and sometimes a liver biopsy to assess the extent of damage.

Common Treatments and Medications for PBC

While there is currently no cure for PBC, treatments aim to slow the disease's progression, manage symptoms, and improve quality of life. The primary medication used is Ursodeoxycholic acid (UDCA), often referred to as ursodiol. This medication helps move bile out of the liver, reducing its harmful effects and can significantly delay the progression to liver failure. It's usually taken long-term, often for life.

For those who don't respond adequately to UDCA, Obeticholic acid (OCA) may be prescribed as a second-line therapy. Other medications may be used to manage specific symptoms, such as antihistamines for itching, artificial tears for dry eyes, and vitamin supplements for nutrient deficiencies that can arise from impaired bile flow. In advanced stages, a liver transplant may be necessary.
